​COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP)

The purpose of a Community Health Improvement Plan (CHIP) is to outline a long-term, systematic effort to improve the health of a community. In Napa County, we will strive to have CHIP strategies and proposed solutions be community-driven and designed with Napa County Public Health staff as support and LHNC partners providing direction and leadership.

​2024-2027 COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP)

 The 2024 -2027 Napa County Community Health Improvement Plan was created through a series of meetings with community partners from Live Healthy Napa County (LHNC) and the Napa County Health & Human Services Agency. These meetings resulted in the adoption of specific goals, objectives, and strategies. During the 2023 Community Health Assessment (CHA) process, the Live Healthy Napa County partners identified five priority areas for improvement.
  • Racial Equity and LGBTQ Inclusion
  • Behavioral Health
  • Housing
  • Access to Health Services
  • Economic Stability
This plan includes the resources and organizations that will be engaged in implementing specific strategies to improve the health and well-being of Napa County community members, including measurable outcomes that will be used to track progress within each priority area. If you are interested in collaborating, please contact [email protected]. ​

Live Healthy Napa County

Live Healthy Napa County (LHNC) seeks to promote and protect health and wellbeing of every member of the community.  LHNC is a public-private partnership to develop a vision of a healthier Napa County and action agenda to realize that vision.
