During a community health assessment (CHA), Napa County Health and Human Services Agency - Public Health Division and community partners gather and analyze health data specific to Napa County. Our community health improvement plan (CHIP) uses CHA data to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measurable health improvement. Our health collaborative then uses the CHA and CHIP data to create a community health action plan (CHAP), which outlines the action plans for achieving health improvements in the community. While a variety of tools and processes are used to conduct a community health assessment; the essential ingredients are community engagement and collaborative participation.

2023 NAPA COUNTY COMMUNITY HEALTH Needs ASSESSMENTThis year, Providence Queen of the Valley Medical Center and Napa County HHSA have partnered to create a shared Community Assessment (CHNA). The CHNA collects qualitative and quantitative health data from across the county in order to create a point-in-time report of community health and determine health priorities for the next 3-5 years. We convened over 230 community members for listening sessions, key informant interviews and prioritization meetings in order to identify 5 health priorities, listed below.

2018 LIVE HEALTHY NAPA COUNTY COMMUNITY HEALTH ASSESSMENTThe Community Health Assessment is an ongoing process to improve the health of our community by addressing the social determinants of health and increase health equity. Together with Live Healthy Napa County partners and community stakeholders, LHNC’s “backbone” support team at Napa County Public Health has launched an effort to conduct a Community Health Assessment (CHA) over the 2017-2018 period.
In keeping with priorities established by Live Healthy Napa County members, this assessment is focused on social determinants of health – those “upstream,” often community-driven factors that can significantly impact “downstream,” individual experiences of wellness and disease. The Community Health Assessment is also designed to shed light on matters of health equity and inequity in Napa County, paving the way for a health improvement planning process (scheduled to begin in late 2018) intended to improve county residents’ health experiences. |
In 2017, the Live Healthy Napa County collaborative identified food security and community livability as priority social health determinants for the 2018 Community Health Assessment. A study of Napa County’s emergency food system (see links to a food study presentation and the food study final report, above) is contributing data to the portion of the Community Health Assessment focused on food security. Community livability continues to be assessed through meetings with Live Healthy Napa County partners and studies of published measures of community health.
Information on both food security and community livability will be compiled into a single health assessment. The goal of the emergency food systems study is to provide recommendations, jointly design next steps, and reduce food insecurity among all residents living at or below 185% of the Federal Poverty Level in Napa County. The Community Health Assessment (CHA) is intended to be a community resource that is used in a myriad of ways. This report begins by outlining specific needs and challenges in Napa County based on the three assessments and provides an overview of resources and strengths as well. The data presented throughout the assessment reflect an understanding that "health" extends beyond the medical setting; thus, to improve health and wellbeing the community strategies must consider the social, economic, behavioral, and structural factors that impact health. |
